A clear affidavit format for the most common declarations
This free affidavit format generator drafts the affidavits people are most often asked for in India: change of name, proof of address, date of birth, a joint affidavit for marriage registration, a gap-year affidavit, a family income affidavit and one for a lost document — or a self-declaration in your own words. Fill in the deponent’s details and the facts, and it writes the opening, the numbered “That…” statements, the verification and the signature lines.
It leaves blank space at the top for the stamp paper or e-stamp certificate and a box for the Notary or Oath Commissioner. It never draws a seal, a stamp or a signature: an affidavit only becomes one when you sign it in front of an authorised officer. Treat the result as a draft to check with an advocate or the notary — this is not legal advice, and offices often prescribe their own wording.
How to use the Affidavit Format Generator
- Choose what the affidavit is for, and add the purpose (for example “submission to the passport office”).
- Enter the deponent’s name, relation, age, occupation and address — and a second deponent for a joint affidavit.
- Fill in the details for that affidavit; anything missing is listed above the preview.
- Read the statements and edit any line — your wording then stays as typed.
- Download the PDF or Word file, print it on the stamp paper, and sign it before the notary.

Affidavit kya hota hai? What an affidavit is
An affidavit (शपथ पत्र) is a written statement of facts that the person making it — the deponent — swears or affirms to be true before an officer authorised to administer oaths, such as a Notary or an Oath Commissioner. The General Clauses Act, 1897 (s.3(3)) says “affidavit” includes an affirmation or declaration where the law allows one instead of an oath.
Because it is made on oath, a false affidavit can be prosecuted as giving false evidence (sections 227 and 229 of the Bharatiya Nyaya Sanhita, 2023, which replaced sections 191 and 193 of the IPC). State only what you know to be true and can support with documents.
How an affidavit is laid out
- Title — “AFFIDAVIT”, often with its purpose.
- Opening — “I, name, son/daughter/wife of …, aged about … years, resident of …, do hereby solemnly affirm and declare as under:”.
- Numbered statements, each starting “That …”, one fact per paragraph.
- The deponent’s signature.
- Verification — the place and date, and that the contents are true to the best of the deponent’s knowledge and belief and nothing material is concealed — signed again.
- Attestation by the Notary or Oath Commissioner, with their seal and signature.
Stamp paper and notarisation
- Affidavits are usually typed on non-judicial stamp paper or with an e-stamp certificate. The stamp duty depends on your state’s stamp law and the purpose, so this tool does not print a value — ask the notary or the office that wants the affidavit.
- Some offices accept a self-attested declaration on plain paper instead; check before buying stamp paper.
- Take an identity proof to the notary, sign in their presence, and keep a photocopy of the attested affidavit.
- A name change for official records usually also needs a newspaper advertisement and, for central records, a Gazette notification — the affidavit is the first step.
Affidavit Format Generator FAQ
What is the format of an affidavit?
Title, the deponent’s details with “do hereby solemnly affirm and declare as under”, numbered “That…” statements, the deponent’s signature, a verification with place and date, and the notary’s attestation. This affidavit format generator lays out all of these.
How do I make an affidavit for a marriage certificate?
Choose “Marriage registration (joint)”: both spouses are deponents and it states the date and place of the marriage, that both were of legal age, that neither had a living spouse and that you are not within prohibited degrees. Check the registrar’s own list of documents, as some states prescribe a form.
Affidavit kya hota hai?
An affidavit (शपथ पत्र) is a sworn written statement of facts, signed before a Notary or Oath Commissioner. It is used as evidence that the facts are declared true by the person who signs it.
Which stamp paper value do I need?
It depends on the state and the purpose, so ask the notary or the office that asked for the affidavit. Many states also issue e-stamp certificates instead of paper.
Can I print the affidavit on plain paper?
Only where the office accepts it. Otherwise print on the stamp paper, or on plain paper with the e-stamp certificate attached as the notary directs.
Does this tool add a notary seal or signature?
No. It leaves blank space for them; the notary attests the affidavit in person.
